Updating Existing Script When App Changes

I'm new to QTP, but have worked with Winrunner for years, so I'll explain an issue and how I handled it with Winrunner:

I would build a winrunner script and, when the app it was testing would change, I would have to update the winrunner script. Sometimes I would be unaware of the change until the winrunner script happened to fail when processing an updated object or window.

Simple enough. From the error, record the newly desired update, comment out any unnecessary code, AND SET THE WINRUNNER INSTRUCTION POINTER TO THE APPROPRIATE LINE OF CODE TO RESUME EXECUTION.

Working with QTP, I don't see how I might start recording from the middle of a script to build the new code, nor do I see how to change my instruction pointer during execution. How do I do these things? Surely we don't have to start recording a new script from the beginning when we encounter a change.
